Please have a photo ID and your insurance cards available.
- In Delaware, a prescription or referral is not required. If you have one, we will add it to your records. A Plan of Care and other communications will be sent to the physician or practitioner you prefer.
- If possible, print out and complete the patient forms. The therapist will have a set of forms as well that they can complete with you on the first visit.
- Have an up to date list of all medications.
